Move 2 Independence (M2I), developed by SASP, is helping people across Somerset get back to living well at home following a hospital stay. By focusing on movement, confidence and personal goals, the service empowers people to return to the activities they want, need and enjoy.
Since its launch, M2I has supported over 500 participants, delivering more than 1,300 face-to-face movement sessions in the home and community. The impact has been significant:
- 82% uptake in the programme
- 97% of participants completed or made progress towards their goals
- 100% rated the service as excellent or good
- 87% reported no falls during their time with the programme
- Confidence to be active doubled, with average scores rising from 3.5/10 to 7.4/10
- Over 60% of participants are no longer classed as inactive, with 40% now meeting national guidelines for weekly activity levels
The service goes beyond numbers, with stories showing how people have regained independence, confidence and enjoyment in daily life - from cooking again after a long hospital stay, to climbing stairs with ease, or even getting back behind the wheel.
Ann was referred to M2I after a hip operation and shoulder injury left her struggling with mobility and daily activities like climbing stairs and washing her hair. These challenges caused her emotional distress and fear about coping independently.
M2I offered personalised support that worked alongside her physiotherapy to provide practical, positive solutions.
“The first time I met you, you were just so practical and positive. Instead of saying ‘we might try this’, you had a perfect answer. You recommended a simple movement for my shoulder, and that changed everything.”
M2I offered:
- Tailored exercises to strengthen her legs, improving her walking and ability to climb stairs.
- Simple, effective movements to ease the shoulder injury.
- Encouragement and confidence building, helping Ann feel more capable and independent.

M2I is designed for people with rehabilitation potential who can actively engage in movement sessions and benefit from personalised, practical support alongside other care. It helps bridge the gap between hospital discharge and long-term independence, reducing falls risk and improving overall wellbeing.
