M2I

What is it?  

Move 2 Independence can support someone with getting back to living well at home, following recent discharge from hospital. Developed by SASP, we help people find ways to move more that work for them to promote good health and happiness. We are pleased to be working with hospitals across Somerset with this service. 

SASP offer movement visits through our Activators in the home along with a range of activity-based resources and programmes that are adaptable, making them suitable for a range of health conditions and symptoms someone may be living with following a stay in hospital. The person is in control of how they use the service to benefit their recovery, but all opportunities are low impact, focus on different types of fitness and may even be enjoyable! 

Who is it for? 

Move 2 Independence is aimed at individuals who demonstrable rehabilitation potential and have left hospital in the last 6 weeks. Participants must be able to actively engage in movement appointments, have stable medical conditions, manageable frailty levels, and a life expectancy that allows them to benefit fully from the rehabilitation process. Those with significant ongoing medical concerns, severe frailty, or a limited life expectancy may not meet the criteria for the programme.

Family & Friends 

Having family or friends provide extra support, guidance and motivation can make a real difference to recovery. SASP would like to include all friends and family in the Move 2 Independence service by offering them the following: 

  1. When your loved one is referred to M2I, a team member will contact them and arrange a first visit. You’ll receive a short video explaining why movement matters, how much activity is safe, what exercises are available, how to know when to increase or reduce activity, and what local support is available. 
  2. You’ll receive a copy of the resource ‘How to have positive conversations about moving more at home resource – This supports our video and is a useful and quick reminder of what can help and why.  
  3. Your assigned Activator will provide you with a direct line of contact with SASP, including phone numbers and an email address for advice and support. 

How to Join:

1. Tell your health professional before leaving the hospital that you’re interested.

 

2. They will review the referral criteria and submit a referral if appropriate.

 

3. Once home, an Activator will contact you to discuss your interest and needs.

Slowly getting towards my goal of being able to walk back around town. But from when we first started I was in so much pain and could hardly walk, compared to now where I am able to walk unaided and mostly pain free.

Bluebird people have been coming and they say ''is there anything I can do for you?'' And my answer is always ''no'', I've already done everything. When the sun comes up, I'm getting up. I'm doing everything for myself, independently. I'm dressing, cooking and eating, showering… I'm doing all that on my own.
