Move 2 Independence for Health Professionals

Move 2 Independence (M2I) can support up to 15 participants per week (approx. 60/month) with functional goals in and outside the home (no transport provided). Referrals beyond capacity will be reviewed individually, and any delays or declines will be communicated.

Inclusion Criteria

• Somerset resident, discharged home within 6 weeks
• Willing and able to engage in movement sessions
• At risk of losing independence without support (mobility, stairs, daily tasks)
• Able to manage personal care independently or with carer support
• Can communicate and follow instructions (family support if needed)
• Participants with a Clinical Frailty Scale (CFS) score of 6 or below

Exclusion Criteria

• Complex/unstable medical conditions or ongoing investigation
• Frequent unexplained falls (refer to CRS)
• Bedbound or needs hands-on support for mobility
• Moderate-severe cognitive impairment or injury-specific rehabilitation needs
• Participants with a Clinical Frailty Scale (CFS) score of 7 or above or requires two-person visits
• Already independent at home
